1. Request your health records.

You have the right to obtain copies of the health records MyoMedics maintains about you. Send a written request — including your full legal name, date of birth, contact information, and a description of the records requested — to info@myomedics.net. Requests are handled under applicable Texas and federal law.

2. Contact the licensing / disciplinary authority.

3. File a consumer complaint.
